Youth Team Participation Consent & Waiver By registering my child for participation in the Rise Up Racing Youth Team program, I acknowledge and agree to the following terms: 1. Assumption of Risk I understand that participation in triathlon, swimming, cycling, running, strength training, and related activities involves inherent risks including but not limited to falls, collisions, equipment failure, weather exposure, and physical injury. I voluntarily allow my child to participate and assume all risks associated with these activities. 2. Release of Liability I hereby release, waive, and hold harmless Rise Up Racing, its coaches, volunteers, staff, sponsors, and affiliated facilities from any and all liability, claims, demands, or causes of action arising out of or related to my child’s participation in the program. 3. Medical Consent In the event of illness or injury, I authorize Rise Up Racing coaches or staff to obtain emergency medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred. 4. Health Acknowledgment I confirm that my child is physically able to participate in endurance sport training and activities. I will inform coaches of any medical conditions, allergies, or limitations that may affect participation. 5. Code of Conduct I understand that Rise Up Racing aims to create a supportive, safe, and respectful training environment. Participants are expected to demonstrate good sportsmanship and follow coach instructions and safety guidelines. Failure to follow these expectations may result in dismissal from the program. 6. Photo & Media Release I grant permission for Rise Up Racing to photograph or record my child during team activities and use these images or videos for promotional purposes including social media, websites, and marketing materials. 7. Transportation & Supervision I understand that parents/guardians are responsible for transportation to and from practices and events unless otherwise stated. I also acknowledge that supervision is limited to scheduled team activities. 8. Refund Policy I understand that program registrations are generally non-refundable unless otherwise stated due to weather, scheduling adjustments, or unforeseen circumstances.
